A Client Information Pack is required to be completed either prior to or during the scheduled "Meet & Greet" session. These can be seen below.  If you have access to a printer, please save these forms to your PC, complete the information required and print off.  If not, we will bring a hard copy to the meeting.  These will need to be completed in full and signed before any services can be carried out.

We also have copies of the staff DBS checks should you wish to have a copy.  Please request a copy at your "Meet & Greet" session.
